TERMS & CONDITIONS OF SALE AND USE

FORM REFORMER STUDIO

These Terms & Conditions of Sale and Use (“Terms & Conditions”) govern the purchase, booking and use of the services provided by FORM Reformer Studio.

By registering, creating an account, making a booking, taking out a membership or purchasing a class pass, the client acknowledges that they have read and accepted these Terms & Conditions.

1. SERVICES

FORM Reformer Studio offers, among other services:

• Reformer Pilates classes;

• Mat Pilates classes;

• Yoga classes;

• Barre, Flexibility and other classes available on the schedule;

• access to the stretching room when available;

• private classes, workshops and occasional events;

• and, where applicable, the sale of drinks, products and accessories.

The schedule, instructors, class times and activities offered by the studio may change.

2. CLASS PASSES AND VALIDITY

Class passes are personal, registered to the individual client and non-transferable.

They may not be resold, transferred or credited to another person’s account unless expressly authorised by FORM Reformer Studio.

The validity periods are as follows:

MORNING BOOST: 1 YEAR

BOOST: 2 MONTHS

IMPULSE: 3 MONTHS

ENERGY: 6 MONTHS

INFINITY: 12 MONTHS

The validity period of all class passes begins on the invoice date, not on the date of the first booking or first class.

The applicable validity period is also indicated at the time of purchase.

ACCESS TO CLASSES

The BOOST, IMPULSE, ENERGY and INFINITY passes provide access to all classes offered by FORM Reformer Studio, across all disciplines.

Clients are therefore free to book any classes available on the schedule, including Reformer Pilates, Mat Pilates, Yoga, Barre, Flexibility and any other disciplines offered by the studio.

These passes allow clients to attend on the day and at the time of their choice, during the week or at weekends and throughout the day, subject to availability and any required class level.

Clients are free to alternate between disciplines throughout the validity period of their pass.

MORNING BOOST

The Morning Boost also provides access to all classes offered by FORM Reformer Studio, across all disciplines.

Its only restriction concerns the times at which it may be used.

Morning Boost credits may be used from Monday to Friday only, for classes scheduled between 7:00 am and 2:00 pm.

They cannot be used for classes scheduled after 2:00 pm or on Saturdays and Sundays.

Morning Boost credits may be purchased individually or in multiples, depending on the client’s needs.

Each credit can be used to book any eligible class across all disciplines offered by the studio.

The Morning Boost is valid for 1 year from the invoice date.

All bookings remain subject to availability and any required class level.

EXPIRY OF CLASS PASSES

At the end of the validity period, any unused credits are permanently lost.

Class passes and credits are not refundable, cannot be exchanged for cash and are not automatically extendable.

An extension or suspension may exceptionally be granted by FORM Reformer Studio in duly justified special circumstances.

Any such decision remains at the studio’s discretion and does not constitute an automatic entitlement.

Promotional offers, introductory passes, gift cards or temporary offers may be subject to specific conditions indicated at the time of purchase.

3. MEMBERSHIPS

FORM Reformer Studio memberships are personal, registered to the individual member and non-transferable.

Memberships provide access to all classes offered by FORM Reformer Studio, across all disciplines.

Members may freely choose between Reformer Pilates, Mat Pilates, Yoga, Barre, Flexibility and any other disciplines offered on the schedule.

Memberships allow members to attend whenever they wish, regardless of the day or time, subject to availability and any required class level.

Members may freely alternate between disciplines throughout the duration of their membership.

Memberships are offered with a fixed commitment period.

The member remains liable for all monthly payments corresponding to the full commitment period, unless otherwise required by mandatory law or exceptionally agreed in writing by FORM Reformer Studio.

Not using the membership, not booking classes or not attending the studio does not suspend the commitment or the obligation to make payments.

4. AUTOMATIC RENEWAL OF MEMBERSHIPS

FORM Reformer Studio memberships are subject to automatic renewal at the end of their term, in accordance with the conditions of the membership selected.

Before entering into the agreement, the member is informed of the commitment period, the renewal arrangements and the conditions for preventing automatic renewal.

Any request to prevent the renewal of a membership must be submitted to FORM Reformer Studio within the period specified when the membership was taken out and before the renewal date.

Once a membership has been automatically renewed for the first time, the member will subsequently benefit from the cancellation rights provided under applicable Belgian legislation governing automatically renewable service contracts.

Any mandatory legal provisions offering greater protection to consumers remain applicable.

5. CANCELLATION DURING THE COMMITMENT PERIOD

Taking out a membership with a commitment period means that all monthly payments remain due until the end of the contractual period.

A request for early cancellation does not therefore automatically terminate the remaining payments.

FORM Reformer Studio may nevertheless consider certain exceptional and duly justified circumstances.

All cancellation requests must be submitted in writing to:

Any exceptional acceptance of an early cancellation does not create a precedent or automatic entitlement for other memberships.

6. CLASS BOOKINGS

Bookings must be made through the booking platform provided by FORM Reformer Studio, including via the website or booking application.

Bookings are subject to availability.

Holding a valid class pass or membership does not therefore guarantee a place in a particular class.

Clients are responsible for checking the date, time, class type and level before confirming their booking.

7. CANCELLING A CLASS

A booking may be cancelled up to 12 hours before the start of the class without losing the corresponding credit.

Cancellations must be made directly through the booking platform.

Any cancellation made less than 12 hours before the start of the class is considered a late cancellation.

In the event of a late cancellation or failure to attend (“no-show”):

• for class-pass holders, the corresponding credit is lost;

• for unlimited-membership holders, the late cancellation or no-show is recorded in the client’s history and the rules relating to repeated late cancellations may apply.

Credits lost as a result of a late cancellation or no-show are not refunded.

8. UNLIMITED MEMBERSHIPS AND BOOKING POLICY

Unlimited memberships allow members to book multiple classes in accordance with the conditions of their chosen membership.

To ensure fair access to classes and prevent places from being unnecessarily blocked, FORM Reformer Studio reserves the right to temporarily restrict a member’s ability to make bookings in the event of repeated late cancellations or no-shows.

In the event of more than three late cancellations and/or no-shows within the same week, access to new bookings may be suspended for 3 days.

This measure is intended to preserve fair access to classes for all members.

9. GUESTS AND DOUBLE BOOKINGS

Class passes remain personal and registered to the individual client. Credits cannot be transferred to another person’s account.

However, a client with a pass containing multiple credits may reserve an additional place for a guest by making a double booking under their own name for the same class.

In this case, one credit is used for the pass holder’s place and one additional credit is deducted for the guest’s place.

Both bookings may therefore appear under the pass holder’s name.

complimentary guest invitation is available only to holders of an unlimited membership, in accordance with the conditions and benefits associated with the membership selected.

Class-pass holders do not benefit from a complimentary guest invitation and must use an additional credit to reserve the second place.

Guests are subject to the same class access, cancellation, lateness, safety, equipment and class-level requirements as all other participants.

10. WAITING LIST

When a class is full, clients may join the waiting list where this option is available.

Joining the waiting list does not automatically enrol the client in the class.

When a place becomes available, clients on the waiting list receive an email informing them that a place is available.

The client must then decide whether or not they wish to attend and must confirm the booking themselves via the booking platform.

Receiving the email does not constitute confirmation of a booking.

The available place remains open until a booking is completed and is therefore not guaranteed to the recipient of the email.

Once the client confirms the booking, the standard cancellation rules, including the 12-hour cancellation period, apply.

11. CANCELLATION OR CHANGES BY FORM

FORM Reformer Studio reserves the right, where circumstances require, to:

• change a class time;

• replace an instructor;

• exceptionally modify the content of a class;

• cancel a class;

• temporarily close all or part of the studio.

This may be necessary due to an instructor’s absence, technical issues, maintenance, insufficient participant numbers, force majeure or any other circumstance preventing the class from taking place as planned.

When a class is cancelled by FORM Reformer Studio, the credit used for the booking is returned to the client.

No credit is lost as a result of a cancellation made by the studio.

The replacement of an instructor does not constitute cancellation of a class and does not automatically entitle the client to a refund or credit.

12. LATENESS AND ACCESS TO CLASSES

Participants are asked to arrive ideally 10 to 15 minutes before the start of the class.

For safety reasons and out of respect for the instructor and other participants, entry may be refused once the class has started.

Where entry is refused because the participant is late, the booking is considered used and the credit will not be refunded.

Participants are therefore responsible for allowing sufficient time for arrival, changing and preparation before class.

13. CLOTHING, GRIP SOCKS AND EQUIPMENT

Appropriate sportswear is required.

Grip socks are compulsory for Reformer classes, as well as for any other activity for which FORM Reformer Studio specifies that they are required.

Participants who do not have grip socks may purchase a pair directly from the studio for â‚¬8.

A personal towel may be required depending on the class or activity.

All equipment provided by the studio must be used carefully and in accordance with the instructors’ directions.

Any deliberate damage or damage resulting from clearly inappropriate use may result in the person responsible being held liable.

14. HEALTH AND FITNESS TO PARTICIPATE

Each participant is responsible for ensuring that their health and physical condition are compatible with the activity undertaken.

Participants must inform the instructor before the start of the class of any injury, pregnancy, recent procedure, pain, medical condition or contraindication that may affect their practice.

If there is any doubt regarding their ability to participate safely, participants should seek advice from an appropriate healthcare professional beforehand.

Information or guidance provided by FORM Reformer Studio instructors does not constitute medical advice, diagnosis or treatment.

Participants agree to follow all safety instructions provided by instructors and to immediately report any pain or discomfort experienced during a session.

FORM Reformer Studio cannot be held liable for consequences resulting, in particular, from relevant medical information not disclosed by the participant, failure to follow safety instructions or inappropriate use of equipment, without prejudice to any liability that cannot legally be excluded or limited.

15. PARTICIPATION OF MINORS

A minor may participate in activities offered by FORM Reformer Studio only when practising at the same time as one of their two parents.

The parent must participate in the class and remain present throughout the entire session.

The minor participates under the responsibility of the accompanying parent.

It is the parent’s responsibility to ensure that the activity is suitable for the minor’s age, physical condition and abilities and to inform the instructor of any relevant information.

FORM Reformer Studio and the instructor reserve the right to refuse or stop a minor’s participation if the conditions required for safe participation are not met.

16. STRETCHING ROOM

Holders of a valid FORM Reformer Studio class pass or membership benefit from unlimited access to the stretching room whenever it is available.

Access depends on the room’s availability, scheduled studio activities and the operational needs of FORM Reformer Studio.

Access to the stretching room therefore cannot be guaranteed at all times.

17. PERSONAL BELONGINGS

Participants remain responsible for their personal belongings while at the studio.

FORM Reformer Studio recommends that participants do not bring valuable items.

The studio cannot be held responsible for lost, forgotten, stolen or damaged personal belongings, except where liability is imposed by law.

18. CONDUCT WITHIN THE STUDIO

All participants are expected to behave respectfully towards other members, instructors, staff and the studio facilities.

Aggressive, insulting, discriminatory, dangerous, inappropriate or disruptive behaviour may result in refusal of access to a class or to the studio.

In the event of serious or repeated inappropriate behaviour, FORM Reformer Studio may take appropriate action, including terminating access to its services, in accordance with applicable law.

19. PAYMENT

The applicable prices are those displayed on the website, application or booking platform at the time of purchase.

Prices are stated in euros and include applicable taxes where required.

Payment must be made using one of the payment methods offered at the time of purchase.

For memberships paid monthly, the member authorises the recurring payments corresponding to the membership selected.

The member is responsible for maintaining a valid payment method throughout the entire commitment period.

In the event of a declined payment or unpaid amount, FORM Reformer Studio may temporarily suspend access to bookings and services until the outstanding amount has been settled.

Such suspension does not cancel any amounts contractually due.

20. REFUNDS, EXTENSIONS AND SUSPENSIONS

Unless otherwise required by law, class passes, memberships, credits and services purchased are not refundable after use or expiry.

Personal absence, holidays, changes in schedule or voluntary non-use of a class pass or membership do not automatically entitle the client to a refund, extension or suspension.

FORM Reformer Studio may nevertheless consider certain exceptional circumstances on a case-by-case basis.

Any extension or suspension granted by the studio is exceptional and does not create an automatic entitlement to any future extension or suspension.

21. GIFT CARDS

Gift cards may be used in accordance with the conditions and validity period indicated at the time of purchase.

Gift cards cannot be exchanged for cash.

Where a gift card is used to purchase a class pass or service, the conditions and validity period applicable to the class pass or service purchased will subsequently apply.

22. PRODUCTS, DRINKS AND ALLERGIES

FORM Reformer Studio may offer drinks, food, accessories or other products for sale, including products supplied by external partners.

Clients are responsible for informing the studio of any known allergies or intolerances and, where necessary, checking the ingredients of products before consumption.

Available allergen information may be requested from the studio before consumption.

23. PERSONAL DATA

Personal data provided to FORM Reformer Studio is processed for the purposes of managing client accounts, purchases, payments, bookings, memberships and communications relating to the studio’s services.

Personal data is processed in accordance with applicable data-protection legislation, including the General Data Protection Regulation (GDPR).

Where certain data is processed by the booking platform or a payment service provider, the privacy policies of those providers may also apply.

24. FORCE MAJEURE

FORM Reformer Studio cannot be held liable for a temporary or permanent inability to provide all or part of its services where this results from an event beyond its reasonable control that could not reasonably have been foreseen or avoided.

Depending on the circumstances and duration of the interruption, FORM Reformer Studio will determine the appropriate measures regarding affected bookings or services, in accordance with applicable law.

25. CHANGES TO THESE TERMS & CONDITIONS

FORM Reformer Studio may amend these Terms & Conditions, in particular to reflect changes to its services, operations or applicable legislation.

The version applicable to a purchase or membership is the version made available to the client at the time the contract is entered into, subject to any changes that may lawfully apply to existing contracts.

The latest version of these Terms & Conditions is available on the FORM Reformer Studio website.

26. ELECTRONIC ACCEPTANCE

By confirming a registration, order, booking or membership and electronically accepting these Terms & Conditions, the client confirms their agreement to the conditions applicable to their purchase.

Clients are invited to read these Terms & Conditions before making any purchase or taking out a membership.

27. GOVERNING LAW AND DISPUTES

These Terms & Conditions are governed by Belgian law.

In the event of a difficulty or dispute, clients are encouraged to contact FORM Reformer Studio first so that an amicable solution can be sought.

If no amicable agreement can be reached, the dispute may be submitted to the competent courts in accordance with applicable law.

OUR CLASSES & STUDIO ACCESS

WHICH CLASSES CAN I ATTEND WITH MY CLASS PASS OR MEMBERSHIP?

All our class passes and memberships provide access to all classes offered by FORM Reformer Studio, across all disciplines.

You can freely choose from the classes available on our schedule, including Reformer Pilates, Mat Pilates, Yoga, Barre, Flexibility and any other disciplines offered by the studio.

You are free to mix and match disciplines as often as you like.

With our standard class passes and memberships, you can attend whenever you wish, regardless of the day or time, subject to availability and any required class level.

The only exception is the Morning Boost, which also provides access to all studio classes but can only be used from Monday to Friday between 7:00 am and 2:00 pm.

BOOKINGS & CANCELLATIONS

HOW DO I BOOK A CLASS?

All bookings can be made directly through our website or booking app.

Simply choose your class, date and preferred time, then use your class pass or membership to confirm your booking.

We recommend booking in advance, particularly for our most popular time slots.

HOW LATE CAN I CANCEL MY CLASS?

You may cancel your booking up to 12 hours before the start of the class without losing your credit.

Cancellations must be made directly through your account on our booking platform.

WHAT HAPPENS IF I CANCEL LESS THAN 12 HOURS BEFORE THE CLASS?

Any cancellation made less than 12 hours before the start of the class is considered a late cancellation.

If you use a class pass, the corresponding credit will be lost.

If you have an unlimited membership, the late cancellation will be recorded in your booking history.

WHAT IF I DON’T ATTEND AND HAVEN’T CANCELLED?

An absence without prior cancellation is considered a no-show.

If you use a class pass, the credit is lost.

If you have an unlimited membership, the no-show is recorded as a late absence.

WHAT HAPPENS IF I HAVE REPEATED LATE CANCELLATIONS WITH AN UNLIMITED MEMBERSHIP?

To ensure fair access to our classes and prevent places from being unnecessarily blocked, if you have more than 3 late cancellations and/or no-shows within the same week, your ability to make new bookings may be suspended for 3 days.

WAITING LIST

HOW DOES THE WAITING LIST WORK?

When a class is full, you can join the waiting list.

If a place becomes available, you will receive an email letting you know that a place is available.

You are never automatically booked into the class.

You can then decide whether or not you would like to book the available place through the booking platform.

IS THE PLACE RESERVED FOR ME WHEN I RECEIVE THE EMAIL?

No.

The email simply informs you that a place has become available.

The place remains available until someone completes the booking, so we recommend booking quickly if you would like to attend.

CLASS PASSES

HOW LONG IS MY CLASS PASS VALID?

Validity depends on the pass you choose:

MORNING BOOST: 1 year

BOOST: 2 months

IMPULSE: 3 months

ENERGY: 6 months

INFINITY: 12 months

The validity period begins on the invoice date, not on the date of your first class.

CAN I ATTEND WHENEVER I WANT WITH MY CLASS PASS?

Yes.

With BOOST, IMPULSE, ENERGY and INFINITY, you are free to choose your days, times and disciplines.

These passes are valid for all classes offered at the studio, during the week and at weekends, throughout the day.

You can therefore organise your practice around your own schedule and freely alternate between disciplines throughout the validity period of your pass.

Bookings remain subject to availability and any required class level.

MORNING BOOST

HOW DOES MORNING BOOST WORK?

The Morning Boost also provides access to all classes at the studio, across all disciplines.

You can use it for Reformer Pilates, Mat Pilates, Yoga, Barre, Flexibility or any other discipline offered on our schedule.

The only difference is the time period during which it can be used.

Morning Boost credits may be used from Monday to Friday only, for classes scheduled between 7:00 am and 2:00 pm.

They cannot be used for classes scheduled after 2:00 pm or on Saturdays and Sundays.

Morning Boost is valid for 1 year from the invoice date.

CAN I BUY MORE THAN ONE MORNING BOOST CREDIT?

Yes.

Morning Boost credits may be purchased individually or in multiples, depending on your needs.

You can therefore choose how many sessions you would like to purchase.

Each credit can be used for any eligible studio class scheduled from Monday to Friday between 7:00 am and 2:00 pm.

VALIDITY, EXTENSIONS & REFUNDS

WHEN DOES THE VALIDITY OF MY CLASS PASS BEGIN?

Your class pass becomes valid from the invoice date.

The validity period does not begin on the date of your first booking or first class.

You can check your pass expiry date directly from your account.

WHAT HAPPENS IF I DON’T USE ALL MY CREDITS BEFORE THEY EXPIRE?

Any unused credits are lost when the class pass expires.

We therefore recommend checking your expiry date regularly and planning your classes sufficiently in advance.

CAN I EXTEND MY CLASS PASS?

Class passes are not automatically extendable.

However, we may consider certain exceptional and duly justified circumstances on a case-by-case basis.

An exceptional extension does not create an automatic entitlement to a future extension.

CAN I GET A REFUND FOR UNUSED CREDITS?

No.

Unused credits are not refundable if they have not been used before the expiry date, except where otherwise required by law or in exceptional circumstances accepted by FORM Reformer Studio.

BRINGING A FRIEND

CAN I BOOK A PLACE FOR A FRIEND USING MY CLASS PASS?

Yes!

If your class pass contains multiple credits, you can make a double booking under your own name for the same class.

The first booking uses one credit for your own place.

The second booking uses one additional credit from your pass for your guest’s place.

It is therefore perfectly normal for both bookings to appear under your name.

HOW DO I MAKE A DOUBLE BOOKING?

First, book your own place as usual.

Then return to the same class, on the same date and at the same time, and make a second booking under your own name using your class pass.

A second credit will be used.

You will then have two places reserved: one for you and one for your guest.

DOES MY FRIEND NEED A FORM ACCOUNT?

No, not when you make a double booking using your own class pass.

Both places can be booked directly under your name.

CAN I INVITE A FRIEND FOR FREE?

Yes, if you have an unlimited membership that includes this benefit.

Complimentary guest invitations are reserved for unlimited-membership holders, subject to the benefits and conditions associated with the membership selected.

Class-pass holders do not benefit from complimentary guest invitations.

However, if you have a class pass, you can still reserve a second place for your guest by using one additional credit from your pass.

DO THE SAME CLASS RULES APPLY TO MY GUEST?

Yes.

Your guest must comply with the same conditions as all other participants, including class-level requirements, arrival times, lateness rules, safety instructions and the requirement to wear grip socks for Reformer classes.

MEMBERSHIPS

HOW DO FORM MEMBERSHIPS WORK?

Our memberships have a fixed commitment period.

When you take out a membership, you commit for the duration corresponding to the membership selected.

Monthly payments remain due throughout the full commitment period, even if you attend less frequently.

WHICH CLASSES CAN I ATTEND WITH MY MEMBERSHIP?

Our memberships provide access to all classes offered by FORM Reformer Studio, across all disciplines.

You can attend whenever you wish, choose your preferred days and times, and freely alternate between Reformer Pilates, Mat Pilates, Yoga, Barre, Flexibility and any other disciplines offered by the studio.

Access remains subject to availability and any required class level.

DOES MY MEMBERSHIP RENEW AUTOMATICALLY?

Yes.

Our memberships are subject to automatic renewal at the end of their term, in accordance with the conditions of the membership selected and applicable Belgian law.

CAN I CANCEL MY MEMBERSHIP BEFORE THE END OF MY COMMITMENT?

Taking out a membership with a commitment period normally means that all monthly payments remain due until the end of the contractual period.

However, we may consider certain exceptional and duly justified circumstances.

WHAT HAPPENS IF I DON’T USE MY MEMBERSHIP FOR A FEW WEEKS?

Not using your membership does not automatically suspend it or the monthly payments.

Holidays, changes to your schedule or periods during which you do not attend the studio do not automatically entitle you to an extension or suspension.

CAN I PAUSE MY MEMBERSHIP?

Membership pauses are not automatic.

Certain exceptional circumstances may nevertheless be considered by our team on a case-by-case basis.

Please contact us if you are experiencing exceptional circumstances.

FIRST CLASS & LEVELS

I’VE NEVER DONE REFORMER PILATES BEFORE. WHICH CLASS SHOULD I CHOOSE?

If you are completely new to Reformer Pilates, we recommend starting with a Light Flow class.

This will allow you to become familiar with the machine, its main settings and the fundamentals of Reformer Pilates before gradually progressing to more advanced classes.

CAN I GO STRAIGHT INTO A MORE ADVANCED CLASS?

We strongly recommend mastering the fundamentals of Reformer Pilates before attending more advanced formats.

Each level is designed with its own pace and degree of difficulty.

ARRIVING AT THE STUDIO

HOW EARLY SHOULD I ARRIVE?

We recommend arriving 10 to 15 minutes before the start of your class.

This gives you enough time to change, settle in and be ready when the class begins.

CAN I JOIN THE CLASS IF I’M LATE?

For safety reasons and out of respect for the instructor and other participants, entry may be refused once the class has started.

If you are refused entry because you are late, the booking is considered used and the credit will not be refunded.

We therefore recommend allowing sufficient time for your journey to the studio.

CLOTHING & EQUIPMENT

WHAT SHOULD I WEAR?

Wear comfortable sportswear that allows you to move freely.

For Reformer classes, we recommend relatively close-fitting clothing so that your instructor can easily observe your alignment and movements.

ARE GRIP SOCKS COMPULSORY?

Yes.

Grip socks are compulsory for Reformer classes.

CAN I BUY GRIP SOCKS AT THE STUDIO?

Yes.

Grip socks are available to purchase directly at the studio for â‚¬8 per pair.

SHOULD I BRING A TOWEL?

A personal towel may be required depending on the class or activity.

We recommend bringing one with you.

HEALTH

SHOULD I TELL MY INSTRUCTOR ABOUT AN INJURY OR HEALTH CONDITION?

Yes.

Please inform your instructor before the start of the class of any injury, pain, recent procedure, pregnancy, medical condition or contraindication that may affect your practice.

CAN I ATTEND CLASSES WHILE PREGNANT?

This depends on the stage of your pregnancy, your previous experience and the advice of your healthcare professional.

Where appropriate, please obtain approval from your doctor or gynaecologist and always inform your instructor of your pregnancy before the class begins.

WHAT SHOULD I DO IF I EXPERIENCE PAIN DURING A CLASS?

Tell your instructor immediately and stop the movement concerned.

You should never continue an exercise that causes unusual pain.

MINORS

CAN MINORS ATTEND CLASSES?

Yes, but only when they are participating in the same class as one of their two parents.

The parent must take part in the class and remain present throughout the entire session.

WHO IS RESPONSIBLE FOR THE MINOR DURING THE CLASS?

The minor participates under the responsibility of the accompanying parent.

The parent is also responsible for ensuring that the activity is appropriate for the minor’s age, physical condition and abilities.

STRETCHING ROOM

DO I HAVE ACCESS TO THE STRETCHING ROOM?

Our class passes and memberships include unlimited access to the stretching room whenever it is available.

Access therefore depends on the room’s availability and the activities scheduled at the studio.

CANCELLATION OR CHANGES BY FORM

WHAT HAPPENS IF FORM CANCELS MY CLASS?

If FORM Reformer Studio has to cancel a class, your credit will be returned to your account.

You will therefore never lose a credit when the cancellation is made by the studio.

CAN AN INSTRUCTOR BE REPLACED?

Yes.

The schedule and instructors may occasionally change due to illness, absence or operational requirements.

The replacement of an instructor does not automatically entitle you to a refund or credit.

GIFT CARDS

HOW DO I USE A GIFT CARD?

Your gift card can be used to purchase a FORM class pass or service.

Once the gift card has been activated on your account, select the class pass you would like to purchase and use your gift card as a payment method.

You can then use the credits from the purchased pass to book your classes.

CAN A GIFT CARD BE EXCHANGED FOR CASH?

No.

Gift cards cannot be exchanged for cash.

PAYMENTS

HOW CAN I PAY?

The available payment methods are displayed when you make your purchase through our booking platform.

Depending on the available options, payments may be made by bank card or Bancontact.

WHAT HAPPENS IF MY MEMBERSHIP PAYMENT IS DECLINED?

If a payment is declined or remains unpaid, access to bookings and studio services may be temporarily suspended until the outstanding amount has been settled.

Monthly payments due under your membership commitment remain payable.
